Anxiety during pregnancy, particularly in the third trimester, may affect maternal psychological readiness and childbirth preparation. Maternal age is considered one of the factors that may influence anxiety levels due to differences in biological and psychological maturity. This study aimed to analyze differences in anxiety levels among third-trimester pregnant women facing childbirth based on maternal age groups at Jatirejo Public Health Center, Mojokerto. This study used an analytical observational design with a cross-sectional approach. A total of 30 third-trimester pregnant women were selected using accidental sampling. Data were collected using the Hamilton Anxiety Rating Scale (HARS) and analyzed using the Mann–Whitney U test. The results showed that respondents in the high-risk reproductive age group had a higher mean rank of anxiety (24.19) compared with those in the healthy reproductive age group (12.34). Statistical analysis showed a significant difference in anxiety levels between the two groups (U = 18.500; Z = -3.560; p < 0.001). These findings indicate that maternal age groups are associated with differences in anxiety levels among third-trimester pregnant women preparing for childbirth. Therefore, early psychological screening and appropriate antenatal counseling are recommended, especially for pregnant women in high-risk reproductive age groups
